GCS = Eye (1-4) + Verbal (1-5) + Motor (1-6); range 3-15
  • Minimum score: 3 (no response in all categories). Maximum: 15 (fully conscious).
  • Severe TBI: GCS 3-8. Moderate: 9-12. Minor: 13-15.
  • Intubated patients are scored as V=1T and documented separately.
